Self defeating client.

Hi can anyone throw some suggestions in here. Ive been working with a client that came for weight loss. Shes 17 stone and has been big all her life. She began by saying she ate very little etc. this denial has continued. Every week she would lose 2lbs then gain it. Suddenly she announced she was giving up bread and pasta as she felt these were the culprits (despite telling me she never really ate these previously). For two weeks she was motivated and lost 12lbs only to revert back to old eating habits which she still denies ever happen and went back to 17stone. After a lengthy discussion and parts therapy session she has admitted she self destructs all her success with diets and is very self defeating. she admits she puts blocks up with regard to exercise also. She had a turbulent childhood which she feels this all stems from and has only just recognised the self punishing behaviour. She has low self esteem (despite coming across confident and determined.) Its taken a long time for her to finally admit she is undoing all the work we do of her own back! She wont do regression therapy as feels she does not want to revisit past. Ive touched on letting go of past etc during sessions and am now concentrating on boosting her confidence. At her suggestion she wanted to stop dieting and just healthy eat. Which is fine, but im wondering how on earth to proceed now! Shall I continue with just confidence boosting?

Im not sure what other therapists think but I feel that your client needs to address the issues from her childhood in some way. Its understandable that she doesnt want to revisit her past but she does need to recognize the triggers that contribute to her eating issue and learn how to control them. Perhaps you could explain to your client (if youre still treating her) that she wont experience and negative emotions attached to her memories and then use a screen projection method to dissociate her from the turbulent events in her childhood, whilst working through whatever issues crop up.

The first thing to remember, is that they would not be your client if they where not self defeating, The second thing to remember is that at this point they are not ready to follow instructions and are wasting your time. In a therapy situation, there can only be one person in change and that person MUST be the therapist. Send her away with instructions not to come back until she is ready to support your efforts and obey, and that next time your fee will be doubled because she lied to you.
